The above 3 fields are required (in the order shown) for the email to work.
You must have the From:, To: and Subject: lines.  Whether you use fields 
after the prompts or pre-filled is up to you.  In other words you can 
also make the heading:

Subject: New Big Bucks Order

After those three lines, the format of the body of the message is up to 
you. Just include the field names from your form surrounded by the 
square brackets [ ]. If your form line is: <INPUT NAME="address" 
SIZE=20>,  then you would include [address] where you wish to see the 
user's data displayed. Here's a sample display:

Hello there, my name is [required-name] and I live in [city].  My address is 
[address] and state & zip code is [state] & [zip].  Please accept my 
order of [amount].
